Nursing at home, or a frail care facility?
Most families who call us are weighing up two very different options, and it's worth setting them side by side.
A facility means moving. A new room, new routines, set visiting hours, and a monthly fee that stays the same whether a great deal of care was needed that week or very little.
In-home nursing means staying put. Your parent keeps their house, their garden, their neighbours and their independence. Family visit whenever they like. And you arrange the hours that are actually needed — which for many households is a few hours a day rather than round-the-clock care.
There's also a quieter benefit that families tend to notice only afterwards: people who stay in familiar surroundings usually stay oriented and engaged for longer. Routine matters, particularly where memory is starting to go.
In-home care isn't the right answer for everyone. Some patients genuinely need constant clinical supervision, and some homes can't practically be adapted. If that's what we find, we'll say so. But the assumption that frailty automatically means a facility turns out, for a great many Johannesburg families, not to be true.
Understanding the level of care you need
Families often call not knowing what to ask for, which is completely normal. Broadly, in-home care falls into three levels:
Registered nurses handle clinical care — wound management, injections, catheters, IV lines, complex medication regimes, and monitoring of unstable conditions.
Enrolled nurses provide nursing care under supervision, suited to patients who are stable but need genuine clinical oversight rather than only daily assistance.
Carers support daily living — washing, dressing, meals, mobility, companionship — where the need isn't primarily clinical.
Many patients need a combination, and the requirement usually changes over time. Someone home from hospital may need a registered nurse for the first fortnight and a carer thereafter. Part of our job is working that out with you, and adjusting it as things change.
